A two day event on 17th & 18th March 2015 for Improving Care for Older People in acute care and Focus on Dementia programmes to demonstrate high quality integrated support and care for older people and people with dementia. Teams will come together to learn from what’s working well and plan how initiatives and ideas could be adapted and applied locally.
